Yes, I give permission for my child to be photographed and/or appear in videos for promotional purposes.

I understand that participation includes physical activities, games, wellness activities, crafts and group events. I voluntarily assume responsibility for my child's participation and release Joy For The Soul Wellness Camp, it's volunteers, staff and sponsoring church from liability except in cases of gross negligence or intentional misconduct.
